Job Responsibilities:
• -Create meaningful relationships over time that motivate patients to reach their healthcare goals
• -Leverage advanced technology to case manage and streamline care coordination between patients, physicians, and the interdisciplinary team
• -Empower your patients to take greater ownership of their health and recovery
• -Following the schedule you help create, you will travel to your patients' homes
• -Pay-per-visit position + paid onboarding stipend pay


Job Qualifications:
• -Active RN (Registered Nurse) state license (PA state)
• -Associate's degree
• -1 yr RN experience
• -BLS and CPR certifications
• -AHA BLS and/or AHA ACLS certifications
