Job Responsibilities:

• -Provide Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning
• -Be involved in patient and caregiver education, supervision, and reporting on patient progress
• -Perform Physical Therapy assessments and develop treatment programs
• -Record and report patient's reaction to treatment and condition changes
• -Instruct patients and caregivers in therapy programs and device use
• -Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
• -Prepare reports and participate in staff and Agency development activities

Job Qualifications:

• -Active PT state license (IL state)
• -Master's degree
• 1 yr experience in healthcare setting
